your html seems valid, if isn't appearing suggest clearing cache , trying again. seek deleting iconcache.db file just-in-case.

note .ico file must real ico file (rather renamed png image) , should contain 16x16, 24x24, , 32x32 sub-images. windows 8 there additional properites can set: http://msdn.microsoft.com/en-us/library/ie/gg491732(v=vs.85).aspx these don't relate icon.
